If you require to book an appointment for the optician to call and perform an eye test simply complete the detail opposite and we will confirm an appointment as soon as possible.

For ongoing eye care we can still visit you at home for your routine eye examination.

We treat all information about you with complete confidentiality, although we may sometimes need to inform your family doctor about our clinical findings during your sight test. We may also need, with your agreement, to refer you to a hospital eye department.

We may sometimes need to make your records available to authorised staff from the PCT for whom we provide services.
